Online Application Process:

The process to apply online requires that applicants first download the Medical Release Forms. You will need to visit your doctor or local clinic to have these filled out and signed by a medical professional.  
When filling out the Application, there will be an upload field at the end to submit your Medical Release. If you were unable to complete the Medical forms but still wish to apply, there is a link below to submit the Medical forms separately. However, you must have the Medical forms submitted to be considered.

The downloadable application is a complete document containing Medical Release, Application, and Acceptance and Agreement. If you choose to download and fill out this document, it must be fully completed. You will still need to have the Medical Release filled out by a medical professional.
